Certification Petition Form (R.4)

INSTRUCTIONS:

  1. Fill in the information in the spaces provided  completely.
  2. Pay (฿50/copy) at Office of  Financial Management.  *If urgent,  consult staff and pay (฿150/copy) at the Office of  Financial Management.  YOUR REQUEST will be ready next day.
  3. Submit the request at Office of Financial Management.
  4. Your REQUEST will be processed usually within a three  working days period.

Note:
Name change  1 copy each

ID Card Petition Form (R.21)

INSTRUCTIONS:

  1. Fill in the information in the spaces provided  completely.
  2. Pay fee at Office of Financial Management for the  Item(s)
    Requested:
    ฿  200 for ID Card substitution.  (for temporary use)
    ฿  100 for ID Card late renewal. (return old ID  Card)

3. Submit the request at Office of the University Registrar.

Official Transcript Petition (R.5)

Financial Management. Write the address legibly on the provided sticker and  submit it with this petition.

fax to  66 2719 1509 Fee is US$  10/copy (including mailing) and pay by Draft, Transfer or Credit Card.
Note:
- Request  will not be accepted if incomplete.
-  Assumption University doesn’t allow name changes on Official Transcript after  graduation.
-  Certification may be issued upon request acknowledging name change.

Resignation Petition Form (R.33)

INSTRUCTIONS:
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Submit the petition form to the Registration Office.
Note:
Student must return ID. Card
In case of  Lost ID. Card, copies of legal police report must be submitted upon request for resignation.
Student ID  beyond 485XXXX do not get refund.

Recommendation Petition (R.26)

INSTRUCTIONS:
Maintain Student’s  Status
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Pay fee 1,000 baht per semester at the Office of  Financial Management.
4. Submit the petition form to the Registration Office.

Change Faculty / Major
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Contact the Dean / the head of a faculty.
4. Pay fee 2,000 baht  at the Office of Financial Management.
5. Submit the petition form to the Registration Office.

Change Address
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Submit the petition form to the Registration Office.
4. Required documents:
A copy of house registration
A copy of ID card
** No fee **

Declare Major / Minor
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Submit the petition form as requested for Major / Minor.
Process within 3 working days
** No fee **

Unofficial Transcript Petition (R.13)
INSTRUCTIONS:  
Payment for Request
Pay ( ฿ 30/copy ) at the Office of Financial Management  and submit the request at the Office of the
University Registrar. Window 2.,  Hua Mak Campus or SM116,Suvarnabhumi Campus.

Name Change / Correction Petition (R.31)
INSTRUCTIONS:
Name/Surname change certificate
Thai Citizen ID / Passport.     
Residence Registration (Thai)
Note:
Assumption University doesn’t  recognize name changes after graduation
Assumption University Certification may be issued  upon request acknowledging name change.

Petition (R.3)
INSTRUCTIONS:
Maintain Student’s  Status
1. Receive the petition form at the Registration Office.
2. Fill in the form completely.
3. Pay fee 1,000 baht per semester at the Office of  Financial Management.
4. Submit the petition form to the Registration Office.

Fee Based Service Guidelines

  • If urgent, 100 Urgent Fee Charged/Petition. Consult staff first.
  • Read and prepare proper petition.
  • Present prepared petition to Office of Financial Management and pay fees.
  • Submit petition and receipt to Office of the University Registrar.
  • Receive further instruction.
